So how does this referral program work? I haven't found any information about referral codes/URLs or anything.
Is the program still active, and if so, how do I refer a new customer and claim my matching plan?

You receive the same plan credited to your hosting account that the referral sign-ups for. This is free for 12 months and you are free to resell this space. After 12 months you can purchase this plan at a 50% discount.
